Supply Change Notification (SCN)
All suppliers are required to notify and receive written approval from ZOLL
prior
to making any changes to the design or manufacture of products ZOLL purchase.
SCNs should be submitted a minimum of 90 days prior to proposed implementation date. End of Life (EOL) notification should be submitted at a minimum of 180 days prior to material last time buy (LTB) dates. Please contact your Purchasing Agent if you have any questions on how to fill out this form.
